LPN – Long-Term Care

Schedule: Part Time evenings (2 or 3 shifts per week)

Assignment Details:

• Contract Length: 22 weeks
• Guaranteed Hours: Part-time hours as specified

Requirements:

• New York LPN Licensure
• BLS certification through AHA or RQI
• 1 year of experience as an LPN
• Experience serving in an LPN capacity at a long-term care facility

Roles & Responsibilities:

• Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.
• Collect and communicate data regarding residents' physical and psychosocial conditions to the nursing team.
• Contribute to the development of individualized care plans that reflect nursing diagnoses and interventions.
• Recommend revisions to care plans based on resident outcomes and collaborate with the healthcare team to address all aspects of resident care.
• Maintain current knowledge and competency in nursing practice and participate in mentoring/orientation of staff.

Compliance / Notes:

• All contracts must run through the holiday season and January.
• Occupational exposure to blood borne pathogens is anticipated, requiring compliance with safety protocols.
