Job Summary: We are seeking a talented and experienced Registered Nurse to join our assessment team as a Minimum Data Set (MDS) Coordinator. As a key member of our care team, you will be responsible for conducting admission/skilled assessments to develop individualized care plans, ensuring our residents receive the highest level of personalized attention and care.
