Registered Nurse Navigator- Oncology Clinic

Salary Ranges: $42.38 $63.57 per hour (DOE)

The Registered Nurse Navigator- Oncology Clinic provides care coordination, patient education, and resource navigation for patients receiving evaluations, diagnosis, surgical treatment, or ongoing management of oncologic conditions within Island Health specialty and surgical services. The navigator serves as a consistent point of contact for patients and families, helping reduce barriers to timely, high-quality, patient-centered cancer care through coordination with internal and external providers.

The Oncology Nurse Navigator collaborates with physicians, advanced practice providers, nursing staff, ancillary departments, social services, community agencies, and regional oncology partners to facilitate seamless transitions across the continuum of care. The position also assists patients with insurance navigation, financial assistance resources, and access to supportive services to help minimize barriers to treatment.

What You Will Be Doing:

• Conducts initial screening and ongoing assessment of patients and families to identify psychosocial, financial, and practical barriers to care.
• Leads and maintains accountability for the oncology nurse navigation program, including development, implementation, evaluation, continuous improvement, and monitoring of program metrics and outcomes.
• Develops, implements, and maintains standardized oncology navigation processes, workflows, and resources in collaboration with clinical leadership.
• Serves as an organizational subject matter expert and resource for oncology navigation; offers clinical guidance, education, and mentorship to nursing and interdisciplinary staff.
• Leads applicable oncology quality, regulatory, and accreditation activities and represents Island Health with regional oncology partners and other external stakeholders.
• Assesses patients for symptom management needs during the course of treatment and communicates with the care team regarding identified needs.
• Utilizes and maintains standardized psychosocial assessment tools to support care coordination.
• Educates patients on treatment plans, side effect management and harm avoidance.
• Provides information and referral services; develops and maintains resource lists for medical equipment, social services, transportation assistance, lodging, support services, and other patient resources.
• Collaborates with pharmacy, financial counseling, billing staff, infusion services, specialty providers, and external oncology partners to facilitate timely access to prescribed therapies and supportive care.
• Works with local and national service organizations, including the Island Hospital Foundation, to coordinate grant funding and patient assistance resources.
• Assists uninsured or underinsured patients in identifying and accessing appropriate financial and community support programs.
• Completes agency-specific referral forms and required documentation in accordance with regulatory and program requirements.
• Participates in interdisciplinary care team meetings and coordinates patient care conferences with internal and external partners.
• Provides leadership and participates in organizational committees, including the Cancer and Surgical Steering Committees when applicable, and contributes to cancer program development.
• Participates in multidisciplinary tumor board conferences, either internally or with regional oncology partners, as appropriate.
• Provides patient education, coping support, and assistance with advance directives as appropriate.
• Documents patient interactions and progress notes in a timely manner, clearly reflecting care coordination activities and patient needs.
• Maintains strict confidentiality of patient information in accordance with organizational policy and regulatory requirements.
• Establishes and maintains effective working relationships with patients, families, visitors, and hospital personnel.
• Coordinates referrals to regional medical oncology, radiation oncology, genetic counseling, palliative care, and other specialty services as appropriate.
• Tracks patients through diagnosis, treatment, survivorship, and follow-up to promote timely care and reduce delays.
• Monitors patients throughout the care continuum to identify delays in care, facilitate timely follow-up, and promote adherence to recommended treatment plans.
• Demonstrates accountability for assigned responsibilities and works collaboratively to meet patient and program needs.
• Performs other duties as assigned.

What You Will Bring to the Role:

• Current Washington State Registered Nurse (RN) license required.
• Bachelor of Science in Nursing (BSN) required; MSN or advanced oncology-related graduate education preferred.
• Minimum five years of progressive oncology nursing experience required, including direct patient care in medical oncology, surgical oncology, hematology, or complex cancer care coordination.
• Demonstrated expertise in oncology navigation, survivorship care planning, and multidisciplinary collaboration across oncology specialties.
• Prior experience in advanced care coordination, symptom management, and supporting patients through diagnostic, treatment, and post-treatment phases strongly preferred.
• Oncology Nurse Navigator certification (AONN+) or equivalent required within one year of hire; existing certification strongly preferred.
• Proven ability to work autonomously, manage complex caseloads, lead patient-centered initiatives, and provide clinical leadership within a general surgery and gastroenterology service line with substantial oncology volume.
• Exceptional communication, critical thinking, and patient advocacy skills required, with a track record of improving patient outcomes and care continuity.
• Demonstrated ability to lead interdisciplinary initiatives, develop effective processes, and use data and outcomes to identify opportunities for program improvement.
• Strong interdisciplinary collaboration and communication skills.
• Demonstrated ability to provide compassionate, professional patient support.
• Ability to work effectively with patients from diverse cultural, socioeconomic, and age backgrounds.
• Computer proficiency required; experience with electronic health records preferred.
