Requirements:

• Current Physical Therapist license in the state of practice
• Experience in skilled nursing facility or geriatric rehabilitation preferred

Roles & Responsibilities:

• Evaluate residents to assess mobility, balance, strength, and functional limitations.
• Develop and implement individualized treatment plans to enhance physical function and reduce pain.
• Provide direct therapy interventions, including therapeutic exercises and gait training.
• Monitor resident progress and adjust care plans as necessary to meet therapy goals.
• Collaborate with interdisciplinary teams in care meetings and contribute to discharge planning.
• Educate residents and families on home exercise programs and safety strategies.
