Description

Hospice Care Center | Day & Night Shift Available

We are seeking compassionate Registered Nurses (RNs) to provide direct patient care in our Hospice Care Center. This PRN/as-needed role is ideal for an RN looking for flexible scheduling while making a meaningful impact for patients and families.

Schedule Requirements

• PRN/as needed
• Must work at least 8 hours per month
• Must work 1 weekend per quarter
• Must work 1 holiday per year
• Shifts: 6:00 AM6:30 PM or 6:00 PM6:30 AM
• Hiring for both Day Shift and Night Shift

Key Responsibilities

• Provide skilled nursing care, symptom management, emotional support, and education to patients and families
• Follow the established Plan of Care and physician orders
• Perform initial and ongoing nursing assessments
• Consult with physicians and/or the Medical Director as needed
• Participate in interdisciplinary team planning and care coordination
• Support admissions, referrals, chart reviews, and patient transfers as needed
• Teach and supervise hospice aides, volunteers, and caregivers
• Order appropriate Durable Medical Equipment (DME)
• Complete timely and accurate documentation in compliance with agency and regulatory standards
• Assist with patient care needs including ambulation, transfers, and positioning
• Maintain clinical competency and perform other duties as assigned

Qualifications

• Graduate of an approved School of Nursing
• Current Ohio RN license
• Minimum 2 years of nursing experience in Medical/Surgical, Oncology, or community health nursing
• Hospice experience preferred
• Valid driver's license required
