Host Home Provider Opportunity

Sample Supports is looking for Host Home Providers to support an adult with disabilities 1:1 in a separate unit located on the guardian's property. This is a unique opportunity where you live on-site and provide individualized care in a home-based setting. Becoming a Host Home Provider in this model allows for a flexible, meaningful career that directly supports a person's independence and quality of life.

Sample Supports will match you with the individual receiving services based on compatibility with your lifestyle, experience, and caregiving approach. This is a specialized living arrangement where you reside in a private, separate unit on the guardian's property, providing consistent support in close partnership with the family. We support a wide range of individuals with varying needs and welcome Host Home Providers of all experience levels who are committed to person-centered care.

People who have experience with disabilities and/or caring for adultswhether professionally or within their own familiesoften thrive in this role. Comfort with living independently while supporting another adult in close collaboration with their family is essential.

Host Home Providers are independent contractors with Sample Supports and receive competitive, typically tax-free payments based on the level of support needed. The individual's needs may include help with activities of daily living, maintaining routines, community engagement, and managing behavioral challenges. Sample Supports provides all required training and certifications, plus individualized training tailored to the person receiving services. Our residential team is available 24/7 to support you in this role.

Sample Supports will provide:

• General training on caregiving and the needs of individuals with disabilities
• Trauma-informed and social emotional care responses
• First-aid, CPR, and general safety trainings for all members of the household
• QMAP (medication administration) training and certification
• Person-centered training based on the person in care
• Integrated and adaptive behavior support plans
• Crisis intervention and emergency response planning
• 24/7 on-call support
• Support team of expert clinicians to provide in-home and community-based training as needed
• Up to 30 hours/week of supplemental programming for the individual if needed
• Trained respite providers and proactive respite scheduling support

Provider duties and responsibilities:

• Provide personal assistance, social and emotional support, and other personal care to adults with intellectual and developmental disabilities in a home and community environment
• Support with activities of daily living including shopping and meal preparation, personal care/hygiene routines, and general in-home support tasks as needed
• Community activities based on the needs of the individual
• Transportation services as needed - may include transportation to/from scheduled activities and appointments
• Complete documentation for services provided using electronic systems
• Medication administration and support as needed

Compensation:

Compensation is typically tax free and varies depending on the level of support the individual needs. Sample Supports offers a competitive rate ranging from $1200 - $5000 per month. Supplemental room and board payments may also be provided at up to $700/month.

Requirements:

• Must be at least 18 years old
• Must be able to lift at least 75 pounds
• Must have dependable and reliable transportation with the ability to safely transport
• Must have a valid driver's license
• Must meet approved motor vehicle record standards for providing transportation for individuals in services
• Must be able to pass Child/Adult Protection Services check
• Due to the nature of the work performed at Sample Supports, and that this position works with an at risk population, we do require a criminal background check be completed.
• Must have appropriate living environment with at least one private bedroom available for placements
• Must be willing to perform Non-Violent Crisis Intervention physical managements as trained
• Preferred knowledge and understanding of developmental disabilities, mental health, and trauma
