Responsibilities

• Provide inpatient pharmaceutical care and ensure safe, effective med use for patients.

• Review and verify medication orders for accuracy, dosing, and interactions.

• Ensure compliance with regulatory standards, policies, and accreditation requirements.

• Collaborate with healthcare teams to optimize therapy and achieve treatment targets.

• Monitor patient outcomes and document medication-related interventions.

• Utilize performance metrics and pipelines to inform daily workflow and targets.

• Provide patient and family counseling on medications, adherence, and side effects.

• Identify opportunities to improve pharmacy processes and efficacy within the unit.

• Assist in formulary management and evidence-based selection of inpatient meds.

• Participate in daily huddles and maintain visibility of clinical activities.

• Support ongoing professional development and seek opportunities for advancement.

Experience/Qualifications

• Active Pharmacist license in the relevant jurisdiction with inpatient experience.

• Inpatient medication order review for accuracy, dosing, interactions, and contraindications.

• Knowledge of regulatory standards, accreditation requirements, and compliance.

• Collaborate with multidisciplinary teams to optimize therapy and treatment targets.

• Utilize performance metrics and pipelines to guide daily workflow and targets.

• Monitor and document patient outcomes; track medication-related interventions.

• Provide patient and family counseling on medications, adherence, and side effects.

• Assist formulary management and evidence-based selection of inpatient meds.

• Participate in daily huddles; maintain visibility of clinical activities.

• Identify and implement process improvements to enhance unit pharmacy efficacy.
