Responsibilities

- Conduct routine assessments, measure vital signs, and report changes to the care team

- Maintain patient safety, assist with activities of daily living, and uphold infection control practices
- Document care activities accurately and communicate with physicians, supervisors, and other team members

- Experience or comfort delivering care in a long-term care setting
