Hospice Team Lead: Patient Care & Care Coordination

VITAS Healthcare in Chicago is looking for a Team Manager to lead the Patient Care Team, ensuring continuity of care and supervising interdisciplinary services. The Team Manager will manage patient admissions, evaluate staff performance, and drive quality improvements while addressing customer service issues. The ideal candidate will have a Bachelor's degree, an R.N. license, and supervisory experience. A solid understanding of primary medical care and palliative care principles is essential.
