Description

• Evaluate patients in both inpatient and outpatient contexts and craft customized treatment plans
• Deliver physical therapy interventions that emphasize mobility, strength, balance, and functional enhancements
• Engage actively with nurses, doctors, case managers, and other rehabilitation team members to ensure optimal patient outcomes
• Educate patients and their families about therapeutic objectives, discharge processes, and home exercise regimens
• Document evaluations, treatments, and progress within organizational and regulatory frameworks
• Contribute to care team meetings and interdisciplinary discussions to uphold coordinated, high-quality care

Education

• Doctorate in Physical Therapy (DPT) from an accredited institution

Certifications

• Active state licensure as a Physical Therapist
• CPR certification is preferred
• Additional certifications (e.g., neurological, geriatric) are beneficial but not mandatory

Skills

• Prior experience is highly preferred
