Nurse Case Management

Summary Join the Indian Health Service and care for diverse American Indian and Alaska Native communities. In this role, you’ll manage complex patient needs, coordinate care, and help improve services in an outpatient setting while making a real impact in a culturally rich and vibrant city. Duties Manage a caseload of patients with chronic conditions by assessing needs and creating individualized care plans. Coordinate care across providers, clinics, and community programs to ensure patients receive timely services and care. Provide education and coaching to patients and families to promote understanding, self‑management, and treatment adherence. Collect and maintain case management data, including service utilization, identified barriers, interventions, and patient outcomes. Use patient data to identify care gaps, improve coordination processes, and support quality improvement efforts. Requirements U.S. citizenship is required. Selective Service registration is required for males born after 12/31/1959. Possible probationary period. Must obtain and maintain medical staff clinical privileges and the required licensure. Background Investigation Requirement This position requires the successful completion of a federal background investigation as a condition of employment. Applicants must complete all required forms within five (5) calendar days of issuance. Financial Suitability Requirement Applicants must demonstrate financial responsibility and maintain good standing with creditors, with no substantial delinquent financial obligations. Qualifications To qualify, your resume must state sufficient education, experience, and licensure to perform the duties of this position. Education: A graduate or higher level degree, bachelor’s degree, associate degree, or diploma from an accredited professional nursing educational program is required. For foreign graduates, official certification from the Commission on Graduates of Foreign Nursing Schools is required. Licensure: Must have passed the National Council Licensure Examination and possess a current, active, full, and unrestricted license or registration as a professional nurse from a state, D.C., Puerto Rico, or U.S. territory. Experience: At least one year of professional nursing experience (equivalent to the GS‑09 grade level) and possession of an accredited nursing degree. At the GS‑11 level, a doctoral degree or one year of professional nursing experience is required. Minimum Qualifications: At least one year of specialized experience equivalent to at least the next lower grade level in the federal service, including case management, coordination of referrals, and development of individualized care plans. Additional Information All federal employees must successfully pass the E‑Verify employment verification check.
