PTA

Provides physical therapy treatments to patients/residents as determined through the therapy plan of care, established by the Physical Therapists, and State Practice Act. Gathers data for screens and reports the results to the supervising Physical Therapist. Records data on patient/resident responses to treatments and communicates to the Physical Therapist overseeing the treatment plan. Documents up-to-date services provided to patients/residents, includes daily progress note, weekly summary, and therapy summary sheet. Participates in conferences, consultations, and educational programs designed to improve patient/resident care and staff performance as directed by the Director of Rehabilitation. Instructs patient/resident, family, and/or caregivers in physical therapy recommendations to be continued upon patient/resident discharge as directed by the Physical Therapist. Participates in scheduled in-service programs related to work topics as directed by the Director of Rehabilitation. Working with the Physical Therapist and Director of Rehabilitation assists in the supervision and education of affiliating students as allowed by State Practice Act. Performs other duties as assigned and allowed by State Practice Acts and regulation. Demonstrates working knowledge of and compliance with the facility Compliance program, policies and procedures, applicable federal and state laws, rules and regulations. Demonstrates compliance with the protection of patient/resident health information and company sensitive information including compliance with company policies and procedures, applicable federal and state laws, rules, and regulations.
