Job Title: RN Hospice Case Manager Profession: RN Specialty: Hospice Duration: 8 weeks Shift: AM Hours per Shift: 8 hours Experience: Prior hospice nursing experience. Strong case management and interdisciplinary team collaboration skills. License: Active/clear RN license (or compact). Certifications: Associates or Bachelors degree. Must-Have: Experience with Homecare Homebase (HCHB). Ability to manage a hospice caseload independently while providing compassionate, patient-centered care. Description: RN Hospice Case Manager needed for an 8-week contract with the option to extend. Schedule is Monday to Friday, 8:00 AM to 4:30 PM, totaling 40 hours per week. Candidates must possess an active and clear RN license. A degree in nursing, either Associates or Bachelors, is required. Candidates should have prior hospice nursing experience. Familiarity with Homecare Homebase (HCHB) is essential. Candidates must demonstrate strong case management skills and the ability to collaborate effectively within an interdisciplinary team. They should be able to manage a hospice caseload independently while delivering compassionate, patient-centered care.
