Location: Chicago, IL Reports To: Chief Medical Officer FLSA Status: Exempt Job Title: Physician, Family Medicine Role Overview As a Family Medicine Physician at Tapestry 360 Health, you will provide comprehensive primary care to patients of all ages, ensuring their physical and emotional well‑being. You will be a critical part of a multidisciplinary team, delivering care that spans prevention, diagnosis, treatment, and follow‑up, with a focus on addressing health disparities in our community. You will also play a key role in community health initiatives aimed at improving overall well‑being through outreach and education. Key Responsibilities Comprehensive Care: Provide patient‑centered primary care, including exams, diagnostics, treatment, and follow‑up, with a focus on prevention and chronic disease management. Team Collaboration: Collaborate with nurse practitioners, physician assistants, and other health professionals to ensure seamless, coordinated care. Leadership & Supervision: Provide clinical supervision to nurses and medical assistants, contributing to the professional development of the healthcare team. Community Engagement: Participate in public health initiatives, addressing issues such as immunization, diabetes management, and hypertension, while ensuring access to care for all community members. Documentation & Compliance: Maintain accurate electronic medical records (EMR) and ensure compliance with healthcare regulations, including privacy standards. Continuous Improvement: Engage in quality improvement activities, including peer reviews and protocol development, to enhance patient outcomes and operational efficiency. Qualifications Education: MD or DO from an accredited medical school, with board certification (or eligibility) in Family Medicine. Experience: Previous experience in a community health setting or working with underserved populations is highly desirable. Skills: Strong interpersonal, diagnostic, and clinical decision‑making skills; familiarity with EMR systems; commitment to high‑quality, evidence‑based care. Salary and Benefits Compensation: The annual salary range for this position is $160,000.00 to $200,000.00, based on experience and qualifications.
