Registered Nurse Position

Responsible for planning and carrying out individualized nursing care based upon accurate assessment and evaluation of the actions performed, according to the treatment plan prescribed by the provider. Provide support and assistance in other departments/sites as needed or directed.

Essential duties and responsibilities include managing patient flow, assessing vital signs, establishing personal rapport with potential and actual patients, developing and implementing educational plans, checking for and/or obtaining signed informed consent forms, assisting with routine examinations and procedures, assessing and referring patients for support services, administering medications, assessing patient health status over the telephone, authorizing medication refills, providing support and assistance in other departments/sites, assisting with maintaining all required tracking within the health center system, functioning within the legal and ethical parameters of nursing care, educating staff members on current knowledge, identifying problem areas with supervisor/health center manager, assisting supervisor/health center manager in seeking and monitoring quality improvement and clinical performance metrics, recommending process improvement/changes, knowing infection control procedures and other safety and security protocols, training new hires/students, working assigned clinical inbox daily, maintaining and tracking all supplies and equipment, and performing other duties as assigned.

Knowledge, skills, and abilities include being professional in appearance and behavior, being customer service oriented and focused, being dependable and punctual, motivating staff, possessing a positive attitude, knowledge of current trends in nursing, being respectful of cultural and religious practices, working effectively independently and within a team and in a leadership role as appropriate, and standard clerical dexterity to operate computers, printers, copiers, scanners, and fax machines.

Education and experience require a degree from an accredited School of Nursing, a current license to practice as a Registered Nurse in the State of Illinois, and maintaining BLS certification.

Comply with all applicable State and Federal laws and understand the expectations and responsibilities of the Patient Centered Medical Home.

Physical demands include the ability to lift up to 50 pounds, assist in patient lifts/transfers utilizing proper body mechanics, stand for extended periods, walk on a regular basis, and sit for extended periods of time.

Work schedule is normally Monday through Friday dayshift unless otherwise specified.
