Your work will not only involve direct patient care but also collaboration with a broader team, optimizing treatment plans to ensure the best possible outcomes.Key ResponsibilitiesConduct comprehensive evaluations to assess patients' physical conditions and create tailored treatment plans based on physician referrals.Implement and modify treatment approaches using evidence-based methods to facilitate optimal recovery, with a focus on various specializations such as orthopedics, pediatrics, neurology, and sports medicine.Deliver skilled therapeutic services, including manual therapy, exercise prescription, and education, specifically targeting improvements in mobility and function.Provide ongoing consultation and support to patients and their families, training them on exercises and care routines to encourage self-management and long-term recovery.Engage effectively in multidisciplinary team meetings to discuss patient progress and collaboratively adjust treatment strategies as necessary.Maintain meticulous records of patient interactions and outcomes to monitor progress and fulfill documentation requirements.Aim to meet key performance indicators related to patient utilization and satisfaction, contributing to the overall objectives of the organization.Patient Population / Client FocusIn this role, you will have the privilege of working with a diverse clientele, including individuals recovering from surgeries, managing chronic pain, or dealing with other health conditions. Our patients often require personalized strategies to regain mobility and independence in their daily lives. The home care setting allows you to tailor interactions and therapeutic approaches that cater specifically to each patient's unique circumstances, fostering a supportive atmosphere for recovery.QualificationsA Doctorate in Physical Therapy (DPT) from an accredited physical therapy program.Current state licensure to practice as a Physical Therapist (DPT).A minimum of two (2) years of clinical experience in various physical therapy settings.Current CPR certification to ensure patient safety during interventions.Exceptional communication skills, both verbal and written, to effectively collaborate with patients, families, and healthcare team members.Proficient in using technology for documentation, scheduling, and communication.A commitment to building therapeutic relationships and helping patients overcome their challenges to achieve measurable goals.Preferred ExperienceWhile your clinical skills are paramount, we value professionals who demonstrate a genuine passion for patient care and community connection.
