As a Nurse (Consultant): Collaborate closely with physicians, advanced practice providers, social workers, therapists, laboratory personnel, and other disciplines to coordinate patient-centered care. Function as part of a nurse and social work case management team, working jointly to address medical, psychosocial, and supportive care needs. Contribute to individualized care planning, patient education, and ongoing clinical assessment to support continuity of care and optimal patient outcomes. Provide direct patient care, including management of multidrug therapy, monitoring for adverse drug reactions, wound care, and prevention of disability.
