If you're passionate about making a difference when it matters most, we invite you to join our dedicated hospice care team.Essential FunctionsProvide direct personal care to terminally ill patientsAssist with personal hygiene, grooming, and toiletingSupport safe mobility, including use of walkers and wheelchairsHelp with meal preparation, feeding, and light housekeepingObserve and report changes in the patient's physical or mental conditionOffer emotional and psychological support to patients and familiesMaintain accurate and timely documentation of visitsAttend team meetings and participate in Quality Improvement (QI) activitiesPractice infection control and follow safety protocols at all timesAbout YouQualifications – What You'll Bring:Active CNA license in the state of employment (Required)Current CPR certification (Required)Driver's license and reliable vehicle with insurance (Required)Six (6) months of direct patient care experience (Required)Comfortable working in home settings, hospice facilities, or long-term care environmentsDemonstrated compassion, patience, and ability to handle emotional situationsWillingness to work independently and adapt to changing patient needsPreferred Background (Not Required):Experience in home health, hospice, palliative care, or oncologyPrior roles in hospital nursing, ICU, geriatrics, med-surg, ER, telemetry, or nursing homesExposure to end-of-life care, wound care, or admissions supportCompensation may vary within the salary range provided based on several factors, including but not limited to a candidate's geographic location, experience, education, skills, licensure, certifications, internal equity, and department equity.
