RN Case Manager - Backfill Position

Schedule

Shift: 8:00 AM - 4:30 PM, Every other weekend required.

Job Description

We are seeking a skilled RN Case Manager for a backfill position focused on acute case management. The selected candidate will be responsible for coordinating patient care, ensuring quality and efficient services through a collaborative approach.

Responsibilities

• Manage and coordinate patient case management for acute care.
• Utilize Meditech, Midas, and Interqual systems effectively.
• Collaborate with a multidisciplinary team to enhance patient outcomes.
• Conduct assessments and develop patient care plans.
• Ensure compliance with regulatory requirements and organizational policies.

Required Experience / Certifications / Licensure

• Acute (hospital) Case Management experience is required.
• Must be knowledgeable in Meditech, Midas, and Interqual.
• Must hold a valid RN license.

Assignment Duration

This is a 13-week contract position.
