Full Time, Day Shift (80 hours biweekly)

About the Team WellSpan Chambersburg Hospital Established in 1895 and located in the heart of Franklin County, WellSpan Chambersburg Hospital has long served its South Central Pennsylvania community with award-winning care. WellSpan Chambersburg Hospital's 274-bed facility provides a community of 156,000 residents full-range inpatient, outpatient and emergent care services. The hospital is noted for its leading-edge technology and full range of medical services, including award-winning heart and stroke care, a cardiac catheterization lab, a family birthing unit and Level II NICU, inpatient rehabilitation, a wound center with hyperbaric oxygen therapy, and surgical services, including robotic surgery. At WellSpan Chambersburg Hospital, we work together to provide our neighbors with important health-related programs focused on disease prevention and management, early detection screening, and tools and resources to prevent disease through health fairs, seminars, children's events, "fun runs," and more. WellSpan Chambersburg Hospital was one of several WellSpan hospitals honored by U.S. News & World Report with the High Performing recognition for 2022-23.

- Assesses needs of population, patient or individual on an ongoing basis. Identifies barriers to care and makes appropriate recommendations for follow-up or referral.
- Educates community members, either individually or in groups, about how to access and/or use the health care system. Shares relevant concerns from community members with medical providers or others within the health care system.
- Identifies and utilizes culturally appropriate outreach strategies to provide specific health promotion and disease prevention education to specific population groups. This may include, but is not limited to: developing and coordinating a network of community lay leaders; implementing community education and screening events, and; conducting relevant follow-up with participants.
- Assists, as requested, in the coordination of care for clients/patients. This may include, but is not limited to: facilitating enrollment in public health insurance and charity care; conducting site visits with at-risk patients/clients, and; providing a cultural context for effective medical care and behavior change as part of a clinical care team.
- Evaluates program outcomes/service effectiveness through tracking participation levels, conducting participant surveys, reviewing chart records, and engaging patient in regular follow-ups.
- Documents and maintains accurate and confidential records of client interventions.
- Actively participates on community coalitions and boards as an advocate for specific populations and as a liaison for WellSpan Health.
- Maintains established policies and procedures, objectives, quality assessment and safety standards.
- Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops.
- Represents WellSpan Health and Community Health Improvement professionally at national, regional, state, and local forums.
- Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.
- Attends meetings as required.
- High School Diploma or GED Required
- 1 year Experience in a community related position. Required
- Basic Life Support Upon Hire Required
- Basic knowledge of disease prevention/health promotion. Upon Hire Required
- Excellent Communication and interpersonal skills.
- Proficient in SBAR technique.
- Ability to effectively present clinical information to the care team.
- Self-motivated and dependable.
- Able to work independently.
- Ability to communicate in Spanish, preferred.
