Required Qualifications:

• Current certification as aNurse Practitioner or Physician Assistant
• Active or eligible licensurein the State of Alabama
• Minimum of four years ofdirect patient care experience in primary care
• Extensive knowledge andexperience in: Chronic disease management, Complex patient care and Palliativecare principles
• Strong organizational,clinical, and problem-solving skills
• Adaptability and comfortworking in a growing, evolving healthcare environment
• Commitment to deliveringcompassionate, patient-centered care

Preferred Qualifications:

• Clinical experience caringfor oncology patients or other high-acuity patient populations
• Strong understanding ofcancer treatment-related complications and supportive care needs
• Experience coordinating careacross multiple providers and healthcare settings
• Background working within aValue-Based Care model
