Pharmacist In Charge - Home Infusion Pharmacy

Salary: $160,000–$175,000 + $5,000–$10,000 bonus
Schedule: Monday–Friday, 8:30 AM–5:00 PM Our client is a leading provider of home infusion therapy and is seeking a Pharmacist In Charge to lead our growing pharmacy team. This position plays a key role in pharmacy operations, clinical oversight, regulatory compliance, team leadership, and the delivery of high-quality pharmaceutical care to home infusion patients. The Pharmacist in Charge will collaborate closely with physicians, nurses, and other healthcare professionals and report to the Executive Team. WHAT YOU'LL DO
• Manage pharmacy operations and ensure pharmaceutical care is delivered in accordance with company policies, accreditation standards, and applicable regulations.
• Lead pharmacy staff, including hiring, training, performance management, and monitoring key performance indicators.
• Evaluate and monitor drug therapy based on the total needs of home care patients in collaboration with physicians, nurses, and other healthcare professionals.
• Ensure compliance with applicable state and federal laws and regulations governing pharmacy practice.
• Maintain accreditation standards and participate in quality assurance activities.
• Coordinate new referrals and patient admissions with nursing, customer service, and distribution teams.
• Review, receive, and verify new patient medication orders and maintain appropriate documentation.
• Partner with physicians and other healthcare professionals to develop patient care plans, establish therapeutic objectives, define monitoring parameters, and prepare progress reports.
• Conduct and document initial and ongoing patient assessments to identify actual or potential drug-related problems.
• Monitor drug therapy outcomes through laboratory results, clinical data, and other relevant patient information.
• Provide medication consultation to patients, families, physicians, and other healthcare professionals.
• Maintain thorough documentation of clinical activities and communications in patient records.
• Process and verify medications, supplies, and equipment prior to dispensing.
• Maintain current clinical knowledge and pharmacotherapy skills through ongoing professional development. WHAT YOU BRING
• Bachelor’s degree in Pharmacy or PharmD required.
• Active Registered Pharmacist license in Iowa.
• Proven experience evaluating and monitoring drug therapies for home care patients.
• Strong organizational and time-management skills.
• Excellent verbal and written communication skills.
• Ability to work collaboratively with physicians, nurses, patients, families, and other healthcare professionals. PREFERRED
• Participation or membership in professional pharmacy societies and organizations. COMPENSATION AND BENEFITS
