Answering and triaging internal and external telephone calls Pulling, filing, autiding, and maintainingi charts for the teams assigned. Closing team charts Co-facilitating team meetings minutes including huddles regarding team-based care, integrated healthcare, and other population health Review and share data reports and outcomes of team services Assist wtih the coordination of care for individuals served on the team Comelting vitals and documenting in E.H.R. Qualifications Education Required High School or better.
