POSITION SUMMARY The Wellness Care Manager works in collaboration with the client, prescriber and other members of the treatment team to ensure coordination, provide education, and assist in reducing barriers to completing the goals addressed in their treatment plan. Title : Wellness Care Manager Location : Freehold, NJ LICENSE/CERTIFICATION Valid New Jersey Driver's License MINIMAL QUALIFICATIONS Bachelor's degree in related field and one (1) year experience working with people preferably in a health care setting PREFERRED QUALIFICATIONS Bachelor's degree in related field and two (2) years of experience working with people in a healthcare setting Experience delivering Motivational Interviewing (MI) with individuals across diverse populations and readiness levels Experience providing Cognitive Behavioral Therapy (CBT) interventions in individual and/or group settings Experience conducting universal screening for substance use disorders, including use of validated screening tools (e.g., SBIRT, AUDIT, DAST, or similar) MAJOR RESPONSIBILITIES/PERFORMANCE OBJECTIVES Collect medical information at client appointments to gather a health history and establish a care plan. Take vital signs and measure height/weight/BMI. Review health screening or assist in completion of health screening and make recommendations based on results in collaboration with supervisor and the client's prescriber Review pain and nutritional screening or assist in completion of these screens and make recommendations based on results Enter allergies in EHR Assist with medication reconciliation in collaboration with supervisor and/or doctor Assist with coordination of linkages and communication with primary care doctor and specialists, as needed. Assist with referrals for specialized care as prescribed Assist with completing, documenting, and reviewing medication fact sheets with clients Educate consumer and/or family on issues ranging from wellness to understanding chronic health conditions, nutritional advice, etc. Coordinate and communicate with other staff involved in client's care as directed by supervisor Provide services using Motivational Interviewing (MI) and Cognitive Behavioral Therapy (CBT) consistent with fidelity standards Conduct universal screening for substance use disorders, ensuring accurate documentation and appropriate follow up Participate in required trainings and supervision related to MI, CBT, and other evidence based practices (EBPs) Demonstrate and maintain proficiency in EBPs through observation, skill demonstration, fidelity assessments or certification processes as required SKILL REQUIREMENTS Possess strong interpersonal skills, flexibility in meeting the needs of consumers and professional interaction with diverse groups of co-workers, external business partners and the community. Demonstrate ability to engage other people, including family members and other supports through a cooperative effort. Ability to follow-up and follow through with attention to detail. Good verbal and written communication skills. Ability to use technology, including electronic health record and a variety of software including Microsoft Office. Ability to travel to multiple locations. PHYSICAL REQUIREMENTS The physical demands described here are representative of those that must be met by the employee to successfully perform the essential functions of this job. The position requires prolonged sitting, some bending and stretching. Good eye-hand coordination and manual dexterity sufficient to operate standard office equipment is required. Employees must have normal range of hearing and eyesight to record, prepare, and communicate information pertaining to this position. The employee must occasionally lift and/or move objects up to 30 pounds.
