A-Line Staffing is now hiring a Full-Time, Field-Based, Home Health Physical Therapist in Jeffersonville, IN! (COUNTIES COVERED ARE WASHINGTON, SCOTT, SOME OF FLOYD AND SOME OF CLARK). PRODUCTIVITY 30 POINTS, MILEAGE REIMBURSED AT $.46/MILE. Job Summary We are seeking a dedicated and experienced Physical Therapist to join a home health team serving Jeffersonville and surrounding counties. This role focuses on delivering high-quality, patient-centered care in a home health setting. Home Health Physical Therapist Compensation The pay for this position is $52/hr mileage reimbursement Benefits are available to full-time employees after 90 days of employment A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates Home Health Physical Therapist Highlights This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs The required availability for this position is Monday-Friday 8am -5pm MINIMUM ONE WEEKEND PER MONTH ON-CALL COVERAGE Home Health Physical Therapist Responsibilities · Perform physical therapy evaluations and treatments based on physician orders/referrals · Conduct comprehensive patient assessments using appropriate tools and procedures · Review patient history, diagnoses, precautions, and contraindications prior to treatment · Develop measurable goals and individualized treatment plans to maximize patient independence and function · Reassess patient progress regularly and modify treatment plans as needed · Communicate patient status, progress, and needs with physicians, care team members, patients, and families · Consult with medical providers regarding evaluation, treatment progress, and discharge planning · Recommend and assist with adaptive equipment to improve patient function and rehabilitation outcomes · Complete timely and accurate documentation, evaluations, treatment notes, and billing in compliance with company, regulatory, and payer requirements · Adhere to HIPAA, corporate compliance programs, professional standards, and all applicable state and federal regulations · Supervise Physical Therapist Assistants and Physical Therapy Aides in accordance with state practice laws · Participate in staff development, meetings, and continuing education to maintain clinical competency Home Health Physical Therapist Requirements · Home Health experience required · Proficiency with HCHB (Homecare Homebase) system · Experience completing OASIS documentation · Strong clinical assessment and treatment planning skills in a home health setting · Active Indiana Physical Therapist License If you think this Home Health Physical Therapist position is a good fit for you, please reach out to me—feel free to call, e-mail, or apply to this posting!aa415a4b-8b21-40fc-a65c-70d2b25ca29a
