Develop and implement individualized treatment plans aligned with each patient’s goals and medical status. Provide direct therapy for communication and swallowing disorders; modify therapy plans based on progress and tolerance. Screen and treat dysphagia; collaborate with the interdisciplinary team to ensure safe, appropriate diet textures and recommended compensatory strategies. Document evaluation findings, treatment plans, progress notes, and discharge summaries in the patient record; ensure compliance with privacy and facility policies. Educate residents, families, and care teams about therapy goals, progress, and strategies to support communication and safe swallowing.
