Job Details Job Location: Langhorne (Bucks), Langhorne, PA 19047. Position Type: Full Time (FT). Salary Range: $100,000.00 - $102,000.00. Shift: Monday‑Friday 8am‑5pm (occasional on call). Territory: Bucks County. About the Role We are seeking dedicated RN Case Managers who provide compassionate, whole‑person care and advocate for patients and families as they navigate hospice care. Deliver attentive, one‑on‑one support to patients and their families, ensuring comfort, dignity, and understanding. Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey. Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure. Administer physician‑ordered medications and treatments with skill and empathy. Educate patients and their families on care techniques and symptom management. Track and communicate changes in patient condition, ensuring timely and effective responses. Keep detailed and up‑to‑date clinical notes to reflect patient progress and quality of care. Collaborate with interdisciplinary teams to ensure comprehensive care delivery. Contribute to supportive duties as part of a compassionate, mission‑driven team. Perform additional duties as assigned. Qualifications A heart for hospice care and a commitment to exceptional service. Valid Registered Nurse license in good standing in the state of employment. Valid driver’s license with reliable transportation. Current CPR certification. Strong observational and communication skills—both written and verbal.
