Immediate opportunity available for a social worker to join our team. The role requires expertise in acute medical hospital social work. Candidates must possess 2 years of relevant experience. Social workers will be involved in assessing patients' needs. Care coordination and discharge planning will be essential functions of the role. Knowledge of advanced directives and protective services is required. Experience in foster care and adoption processes is desired. Social workers will work with a diverse patient population, including neonates, pediatrics, adults, and geriatrics. Participation in care conferences and collaboration across disciplines is expected. Weekend shifts will be part of the rotation to meet department needs. Strong documentation skills are essential for compliance with care standards. Candidates must be prepared to float between facilities as needed. Adherence to organizational policies and procedures is mandatory.
