The Division of Internal Medicine within our organization is on the lookout for a full-time, seasoned Nurse Practitioner to become part of our outpatient academic clinic. This position is dedicated to providing exceptional primary care to a well-established patient base while collaborating closely with physicians, pharmacists, and other members of a multidisciplinary care team.

This role presents a fantastic opportunity for a Nurse Practitioner who prioritizes teamwork, continuity of care, and engagement in educational and quality enhancement initiatives.

The Nurse Practitioner will deliver comprehensive primary care services to adult patients in an outpatient Internal Medicine environment. The focus will be on evidence-based practices, patient-centered care, and collaboration within an academic setting.

• Deliver thorough primary care services, including assessment, diagnosis, treatment, and ongoing management of both acute and chronic conditions.

• Conduct patient consultations, order and interpret diagnostic tests, and prescribe medications as permitted by the scope of practice.

• Oversee preventive care, health maintenance, and patient education initiatives.

• Work closely with attending physicians, nursing personnel, and care coordinators.

• Engage in interdisciplinary collaboration, including quality improvement projects with the pharmacy team.

• Accurately document care in the electronic medical record in a timely manner.

• Foster a culture of safety, quality, and continuous improvement.

• Participate in academic activities, including the potential to mentor Nurse Practitioner students.

• Master's or Doctoral degree from an accredited Nurse Practitioner program.

• Current license and active registration as a registered nurse/nurse practitioner in New York State.

• Previous experience in primary care or Internal Medicine is highly preferred.

• Strong clinical, communication, and organizational abilities.

• Genuine interest in academic practice, teaching, and collaborative care models.
