Role Description This is a full-time, on-site role for a Wellness Coordinator/Case Manager located in Yorktown Heights, NY. The Wellness Coordinator/Case Manager will oversee and implement health and wellness programs, provide health promotion education, coordinate care plans, and support residents in achieving their wellness goals. Additional responsibilities include maintaining communication with residents, families, and team members to ensure quality service and fostering a positive living environment for residents. Qualifications Knowledge and skills in Health & Wellness and Health Promotion Strong Customer Service and Communication skills to address resident needs and liaise with families Experience or interest in Wellness practices and initiatives Compassionate and empathetic approach to resident care Organizational and problem-solving abilities to manage care plans effectively Relevant certifications or experience in geriatric care, social work, or a similar field is preferred #J-18808-Ljbffr Meridian Senior Living
