LPN Assisted Living

The LPN Assisted Living is a key player in our Resident Care Team. They assists our residents by administering medication and performing routine nursing care and related duties as required.

Responsibilities

• Administers medications to assigned residents according to professional standards, facility policy, and physician's orders. Documents response to medication and medication effectiveness as required.
• Ensures the medication cart is cleaned and stocked daily with paper supplies.
• Ensures medication orders are picked up and medication is ordered from the pharmacy and delivered in a timely manner.
• Performs nursing procedures/treatments to assigned residents according to professional standards, facility policy and physicians orders. Documents response and effectiveness of treatments. Includes vital sign, specimen, and weight collections.
• Communicates accurate, up-to-date information to Resident Care Assistants and Memory Care Partners. Provides direction, feedback, and assistance to RCAs and MCPs as needed.
• Produces timely accurate and legally compliant documentation of residents' status, changes, and observations in resident chart and communication book.
• Educates resident/family members regarding plan of care, medications, procedures and treatments, and participates in Family Meetings as necessary.
• Provides continual monitoring of resident's physical, social, and mental status. Promptly reports changes and concerns to LPN Manager.
• Maintains and develops work-related knowledge and skills through continuous learning activities, attendance at in-services and workshops, etc.
• Completes special projects/tasks in support of effective department functioning as assigned by supervisor.

Qualifications:

Experience

• 2 years professional nursing experience
• previous experience working with senior population
• experience in Assisted Living is preferred

Education

• High School Diploma/GED
• Current NYS LPN licensure
• Current CPR certification
