RN Case Manager: Discharge & Care Coordination
Advanced Health Care
Advanced Health Care seeks an RN Case Manager to support the Director of Nursing and facilitate smooth, safe discharges for residents. You will coordinate with the IDT to set discharge dates, complete paperwork, arrange DME and transportation, and educate families. This full-time, on-site role requires strong communication, confidentiality, and knowledge of resources for older adults. Weekend/holiday coverage is occasional. #J-18808-Ljbffr
