Mobile Physical Therapist

At Vitality Health Care, we believe exceptional therapy begins with meaningful relationships. We are seeking a compassionate, motivated, and skilled Physical Therapist to join our growing mobile rehabilitation team. As a Mobile Physical Therapist, you will provide one-on-one care in patients' homes, helping them regain mobility, improve function, reduce pain, and maintain their independence.

What You'll Do

• Perform comprehensive physical therapy evaluations in patients' homes.
• Develop individualized, evidence-based treatment plans based on each patient's goals and functional abilities.
• Provide skilled interventions to improve strength, balance, mobility, endurance, flexibility, and overall function.
• Educate patients and caregivers on home exercise programs, fall prevention, body mechanics, and safe use of assistive devices.
• Regularly reassess patient progress and modify treatment plans to maximize outcomes.
• Complete timely, accurate documentation including evaluations, daily treatment notes, progress reports, and discharge summaries.
• Collaborate with physicians and other healthcare professionals to coordinate high-quality patient care.
• Build trusting relationships with patients and families through compassionate, patient-centered care.
• Maintain compliance with Medicare Part B regulations, state practice acts, and agency policies.
• Stay current with evidence-based practice through continuing education and professional development.

Qualifications

• Doctor of Physical Therapy (DPT) from an accredited program.
• Current New Jersey Physical Therapist license in good standing.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Strong clinical reasoning and independent decision-making skills.
• Excellent communication and interpersonal skills.
• Experience in home health, outpatient orthopedics, geriatrics, or neurological rehabilitation is preferred, but motivated new graduates are encouraged to apply.
