We CARE for our patients like they are our own FAMILY.Schedule: Monday–Friday | 8:00 AM–5:00 PM (with on call requirements)** $7,500 Sign On Bonus**Genesis Home Care is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in the comfort of their homes.What You'll DoProvide skilled nursing care according to physician orders and RN-developed plans of careAssess and monitor patient conditions and communicate changes to the RNCMAdminister medications and treatments as prescribedEducate patients and families on disease management, medications, safety, and preventative careInvolve patients and families in their plan of careComplete accurate and timely clinical documentation in accordance with agency and Medicare requirementsCommunicate with the care team and scheduler regarding patient needs and schedule changesParticipate in on-call coverage, case conferences, in-service training, and required meetingsProvide care within the LPN scope of practice and applicable state and federal regulationsTravel to patients' homes within the assigned geographic areaWhat We're Looking ForCurrent, active LPN license in the state of OhioMinimum of 1 year of acute care LPN experience required; 2 years preferredHome health experience preferredStrong communication and interpersonal skillsCompassionate, patient-focused approachAbility to work independently, prioritize responsibilities, and make appropriate nursing judgmentsStrong attention to detail and reliable work ethicAbility to work effectively as part of a teamValid driver's license and auto insurance with your name listed as a driverEducation & CredentialsGraduate of an approved practical nursing programCurrent, active LPN license in the state of OhioJoin Genesis Home Care Make a difference by providing meaningful, one-on-one care to patients where they are most comfortable — at home!We'll help you put your passion for patient care to work.
