Key Responsibilities

• Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions
• Meet established department objectives while delivering high-quality patient care
• Serve as co-leader in an interdisciplinary approach to patient groups
• Provide home health physical therapy services, including travel to patient locations

Required Qualifications

• Doctor of Physical Therapy (DPT) degree required if license was obtained on or after January 2016, or foreign equivalent
• Active Physical Therapy licensure in all applicable states of practice
• Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification from an approved vendor
• Valid Driver's License required; must be at least 21 years of age or older

Preferred Experience / Skills

• Prior experience in a home health or community-based physical therapy setting
• Experience working within an interdisciplinary care team environment
