RN, Primary Nurse PACE

This position is part-time in the PACE Primary Care - Nursing Department, located at 225 Sumner Street - Lewis Mall. Position Summary As a nurse in our Neighborhood PACE Program, you will be part of a dynamic interdisciplinary team and work in an innovative model of care with proven positive impacts on helping older adults stay healthy, independent and active in a community health setting. The Program of All-Inclusive Care for the Elderly (PACE) is one of the longest standing nationwide and is part of one of the largest community health centers in the country. Responsibilities Assess a patient through obtaining history, performing appropriate tests, making or collecting observations from the participant and caregivers, and consulting with other members of the IDT. Prepare and present a patient for rapid provider review and treatment. Arrange for or directly implement care plan interventions and testing, treatment, or other provider orders for a panel of participants. Oversee delegated tasks to ensure completion, monitor outcomes, and make adjustments to the care plan as needed. Safely and effectively administer medications, tests, and treatments as ordered. Manage the refills of all patient medications with appropriate authorization by the provider. Teach patients and families health care regimens. Explain and provide information regarding disease and wellness, tests, results, medications, diet, exercise, and other aspects of the plan of care to patients. Qualifications Baccalaureate degree in nursing preferred. Current Massachusetts license in nursing. Current BLS certification required. Minimum of 2 years’ experience in ambulatory, home health, hospital or sub‑acute setting preferred; equivalent training or related experience considered. A minimum of one year experience working with a frail or elderly population. Skills and Abilities Basic physical assessment and triage skills. Ability to communicate assessment and triage findings efficiently, effectively and accurately in verbal and written formats. Ability to communicate effectively with professional and non‑professional staff, outside providers, participants and families in person and by phone. Bilingual a plus. Excellent customer service skills. Flexibility, initiative, creativity and excellent organizational skills. Basic computer skills including experience with word processing and email; must be able to efficiently and effectively navigate windows‑based programs and NeighborHealth’s EMR.
