Physical Therapist

Join as a Physical Therapist to deliver personalized, in-home and community-based care to older adults. Responsibilities Include Providing one-on-one physical therapy in private residences and senior living communities. Performing evaluations, developing individualized treatment plans, and delivering evidence-based therapy. Supporting patient wellness, mobility, independence, and safety. Documenting care accurately within electronic medical records. Collaborating with an interdisciplinary team of healthcare professionals. Educating patients, caregivers, and staff to promote aging in place. Utilizing telehealth and virtual care to enhance access and flexibility. Qualifications & Skills Valid Physical Therapy license or eligibility. Degree from an accredited program. Proficiency in basic computer use and EMR systems. Current CPR certification. Physical Requirements Ability to exert moderate physical effort, including lifting up to 50 lbs. Assistance with patient transfers, repositioning, and mobility activities.
