Hospice RN Case Manager Join our dedicated team at Hospice Services of Massachusetts, where we strive to make a lasting difference in the lives of patients diagnosed with a life-limiting illness. We are seeking a compassionate and experienced Registered Nurse (RN) to serve as a Hospice Case Manager in our Lakeville, Massachusetts location. Job Summary As a Hospice RN Case Manager, you will plan, organize, and direct hospice care to meet the needs of individuals and families within their homes and communities. Weekly pay range of $42 to $52 an hour, depending on experience. Responsibilities Supervise hospice aides Complete comprehensive assessments of patients and families to determine hospice needs Provide professional nursing care by utilizing all elements of the nursing process Assess and evaluate patient status, writing and initiating plans of care, and regularly re-evaluating patient and family needs Develop care plans that establish goals, based on nursing diagnosis, and incorporate palliative nursing actions Counsel patients and families in meeting nursing and related needs Provide health care instructions and assist patients with activities of daily living Act as Case Manager when assigned by Clinical Supervisor, coordinating patient care for assigned caseload Requirements Graduate of an accredited school of nursing One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home care Current licensure in the State and CPR certification Bachelor's degree, with one (1) year of hospice care experience preferred What we offer Competitive and Weekly Pay Holiday Pay for Hourly and Salaried Employees Mileage Reimbursement Overtime Pay for Hourly Employees Career Advancement Opportunities Exclusive Employer Discount Program Available for Eligible Team Members Employer-Paid Life Insurance 401(k) with Employer Match Personal Days Vacation and Personal Time Health, Dental, and Vision Insurance We are an equal opportunity employer that values diversity at all levels.
