Heather Hill Nursing Center Full-Time Occupational Therapist (OT) – New Port Richey, FL
Heather Hill Nursing Center is excited to offer an opportunity for a Full-Time Occupational Therapist (OT) to join our in-house rehabilitation team in New Port Richey , FL . This role is ideal for a clinician who values patient-centered care and thrives in a collaborative skilled nursing environment.
Our in-house therapy model supports consistent care, strong interdisciplinary collaboration, and individualized treatment plans focused on improving residents’ independence and overall quality of life.
Position Overview
As an Occupational Therapist , you will evaluate residents, develop individualized treatment plans, and provide skilled therapy services to improve functional abilities, safety, and participation in daily activities.
Key Responsibilities
• Evaluate residents to identify functional impairments and therapy needs
• Develop and implement individualized treatment plans focused on ADLs and IADLs
• Monitor resident progress and adjust treatment plans as needed
• Recommend adaptive equipment and environmental modifications
• Educate residents, families, and caregivers on therapeutic techniques
• Maintain accurate and timely documentation per facility and regulatory guidelines
• Collaborate with the interdisciplinary team to ensure optimal outcomes
• Maintain compliance with all facility, state, and federal regulations
Qualifications
• Active Florida Occupational Therapist (OT) license
• In good standing with all licensing boards and regulatory agencies
• SNF or long-term care experience preferred
• Familiarity with Net Health documentation system a plus
• Strong communication and clinical skills
