Position Details
Patient Population: Adult and Geriatric patients enrolled in the PACE (Program of All-Inclusive Care for the Elderly) program
Setting: Primary Care Clinic with in-home visits as needed
Responsibilities:
- Provide comprehensive primary care, including diagnosis and treatment of acute and chronic conditions
- Perform preventive care, urgent care, and comprehensive health assessments
- Evaluate patients in the clinic, PACE Day Center, and their homes
- Collaborate with the interdisciplinary PACE team and Chief Medical Director
- Coordinate referrals to community specialists and providers
Required Procedures: Primary care evaluation and management, preventive care, chronic disease management, health assessments
EMR System: Not Listed
Support Staff: Interdisciplinary PACE care team
Days Per Month: Full-time (4x10 schedule)
On-Call/Weekends: None
- Current unrestricted Colorado license
- National certification as FNP, AGPCNP, or appropriate population focus
- DEA registration (or eligibility) and controlled substance license
- Current CPR/BLS certification, valid driver's license, and minimum 2 years of experience
