Case Manager

Reports To: Case Management Director Supervises: N/A Minimum Qualifications: High school diploma or GED Two (2) years of business office experience Proficient in Microsoft Word, Excel, and Outlook Valid Texas Driver License Clean driving record with demonstrated safe and responsible driving habits Knowledge & Skills: Ability to form and maintain positive relationships with residents, staff, and school personnel Ability to adapt quickly to changing environments Effective communication and organizational skills Some medical knowledge to support clinical and administrative operations Basic Function: Provide administrative and logistical support to the Case Management Director. Responsibilities include scheduling medical and psychiatric appointments, transporting residents, preparing documentation, and supporting treatment-related processes. Note: This position is subject to mandatory drug testing. Any offer of employment is contingent upon satisfactory drug screen results. Status: Exempt (salaried, not eligible for overtime). May require more than 40 hours per week. Key Responsibilities and Tasks: Immediately report any suspected abuse or neglect to the Hotline and Executive Director or designee. Support the Case Management Director with administrative and logistical tasks. Schedule and/or assist in coordinating: All Medical Appointments Caseworker, legal, CASA, and family visits (in person and virtual) Transport residents to on- and off-campus appointments. Some travel may occur outside of Kerr County. Safely transport children using agency-provided vehicles. Coordinate and supervise parent-child-sibling communication including phone calls, in person visits and virtual visits. Collaborate with Star health to complete Star Health treatment plans, reviews and required assessments Communicate and Collaborate with all parties involved in the child’s case to include the Case Worker/Case Care Specialist, Attorney, CASA, and Kinship/Parent/Guardians Attend Psychiatric Clinics, ARDs and all medical appointments for assigned children. Maintains children’s records in the Electronic Record System Assist in completing Resident Data Forms and Resident Contact Forms. Review and manage resident correspondence (incoming/outgoing). Support the Preparation for Adult Living (PAL) Program for residents over 16 years of age. Collaborate aftercare service with child’s next placement Demonstrate professionalism, maturity, and serve as a positive role model in alignment with HCYR policies and code of conduct. Fulfill professional development requirements, including CPR/First Aid and other HCYR/TDFPS/T3C-mandated trainings. Address behavior issues using HCYR and TDFPS-approved methods, including TBRI principles. Model responsible, mature behavior in all interactions; follow HCYR code of conduct and policies. Work hours may extend 40 hours per week, based on organizational needs. These additional hours are included in the salaried compensation.
