Patients are referred to Priority Care by their primary care provider, a specialist, or after a hospitalization. Once enrolled, they transition to being managed by our Priority Care team rather than their PCP. The program includes frequent provider visits, comprehensive case management, and access to additional resources such as pharmacy support, community resource advisors, and hospice coordination as needed. As a physician in the Priority Care Department, you will play a critical role in stabilizing and educating patients on their conditions, with the ultimate goal of transitioning them back to their primary care provider with the knowledge and tools needed to manage their health effectively. You will work alongside a dedicated team, including nurse case managers who maintain regular contact with patients, ensuring continuity of care between visits. Our approach prioritizes patient education, proactive care, and resource coordination to reduce hospitalizations and emergency visits while enhancing patient independence. This is an opportunity to make a meaningful impact on some of our most vulnerable patient populations by providing high-quality, patient-centered care. We are looking for a physician who is passionate about chronic disease management, patient education, and multidisciplinary collaboration. Our Providers see 14 patients per day and work with Medical Assistants and a team of nurses to manage the patient care.
