Home Health RN Case Manager — Growth & Benefits

Angels Care Home Health is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of in-home patient care. You will perform admission visits, assess eligibility for home health services, and develop comprehensive plans of care tailored to each patient. Qualifications include a nursing degree, active RN license, valid driver's license, and CPR with Hepatitis consent/declination.
