Nutrition Assistant - Home Delivered Meals Assessor

Job Title
Set appointments to assess and reassess older adults for the Home Delivered Meal program. Track all data in computer system as necessary for grant requirements. Refer clients and caregivers to other services and agencies as necessary. Work with staff to set up meal delivery. Communicate effectively with families, caregivers and older adults. Other duties as assigned.
Previous work experience: Experience working with older adults or food service helpful.
Education: High school diploma or equivalent preferred.
Special skills and training: Must be organized and communicate effectively with older adults, caregivers, volunteers and staff. Good driving record and basic computer skills required.
Start date and work schedule: Starting 10/26/26 28 hours per week, Monday - Thursday 8:30am - 3:30pm.
