Expand Access to Home-Based Care in the Yonkers Community

@Home Therapy is seeking an Occupational Therapist to provide in-home services to adult patients throughout Yonkers. This position focuses on supporting individuals who need functional rehabilitation to remain safe and independent in their homes.
This location offers enhanced earning potential and is ideal for clinicians looking to build a consistent, community-based caseload.

Compensation & Incentives:

• $500 bonus after 50 completed visits.
• Additional earning incentives available for this service area .

What You’ll Be Doing:

Functional Home Assessments:

• Evaluate patients' ability to safely and independently perform activities of daily living (ADLs) and instrumental activities of daily living (IADLs) within the home.
• Identify environmental, physical, and cognitive barriers affecting safety, independence, and routine function.

Skill Restoration & Training:

• Provide therapy to improve performance with self-care activities such as dressing, grooming, bathing, toileting, and meal preparation.
• Teach adaptive techniques and energy conservation strategies to promote independence in daily routines.

Therapeutic Interventions:

• Develop and implement individualized treatment plans based on patient goals and clinical findings.
• Recommend and train patients in the use of adaptive equipment and home modifications to improve safety and functional performance.

Caregiver & Patient Education:

• Educate patients and caregivers on safe assistance techniques, adaptive strategies, and home safety practices.
• Collaborate with caregivers and the interdisciplinary care team to support continuity of care.

Documentation & Compliance:

• Complete timely documentation using the agency's electronic medical record (EMR) system.
• Maintain compliance with clinical, regulatory, and agency documentation standards.

Qualifications:

• Active New York State Occupational Therapist license.
• Experience with adult or geriatric populations preferred. 
• Strong clinical reasoning and adaptability in home environments. 
• Ability to work independently in a field-based role .
• Effective communication and patient engagement skills.
