Position Duties Include:

• Visiting clients in their homes and helping them maintain their independence. There really is no place like home! You'll be helping families in your community stay in their homes.
• Provide Direct Support to individuals this includes supporting the client in individualized goals, including personal care tasks such as helping client get in and out of bed, assisting with bathing/showering, and making sure their home is clean and safe.
• Building relationships with individuals and reporting any concerns to the Designated Coordinator.
• Supporting community engagement by attending community events, etc.
