Nurse Practitioner: Collaborative Care Clinician

Advocate Health is seeking an experienced registered nurse and/or advanced practice nurse for a 0.8 FTE position at the Union Grove Clinic. The role focuses on delivering high-quality patient care, coordinating with a multi-disciplinary team, and supporting clinic operations. Master’s in Nursing and APN licensing are required, with DEA registration and BLS/ACLS as applicable. This is a full-time in-clinic role with a strong emphasis on documentation, safety, and collaborative practice within our #J-18808-Ljbffr 223 Aurora Medical Group, Inc.
