Services are designed to assist the individual in acquiring, retaining, and improving self-help, socialization and adaptive skills necessary to reside successfully in-home settings. These services are provided in the person’s home and in the community. Provide emotional, medical, physical, vocational, social, and educational support designed to meet the individual’s IP/ISP. Support the individual in developing everyday life skills and independence. Assist with self-help, socialization and adaptive skills in home and community settings.
