Home Health Licensed Vocational Nurse (LVN) - PRN

Flexible Schedule | One-on-One Patient Care | Supportive Team Environment

BrightStar Care is seeking compassionate and skilled Licensed Vocational Nurses (LVNs) to join our growing home health team. If you enjoy building meaningful relationships with patients, working independently, and delivering high-quality care in the home setting, we'd love to connect with you.

This role offers flexible scheduling, consistent client opportunities, and the chance to make a meaningful impact while being supported by an experienced clinical team.

What You'll Do

Provide skilled nursing care in patients' homes
Deliver individualized care based on established care plans
Monitor patient condition and report changes to supervising clinical staff
Educate patients and families on basic health maintenance and care needs
Maintain accurate documentation and charting
Collaborate with the BrightStar Care team to ensure quality patient care

Qualifications

Active LVN license
Home health, private duty, hospice, skilled nursing, or acute care experience preferred
Strong clinical, organizational, and communication skills
Ability to work independently and manage time effectively
Reliable transportation and willingness to travel within assigned service area
A commitment to compassionate, high-quality patient care
