Hospice Intake Coordinator (LPN/MSW) Admissions & Care Flow

Brighton Hospice

Brighton/Suncrest Hospice in Minnesota is seeking a Clinical Intake Coordinator to handle admissions inquiries, referrals, and documentation workflows. You will support scheduling goals and coordinate with the sales team to ensure a smooth intake process.
